Maine Health Access Foundation: Healthy Community Grants Program

The Maine Health Access Foundation is committed to improving the health of everyone in Maine, especially people who are uninsured and medically underserved. The focus of the Foundation’s Achieving Better Health in Communities: Healthy Community Grants Program is on supporting place-based, collaborative, community-led efforts that can help transform communities into supportive environments that enable people to live healthier lives. The Foundation’s current request for proposals (RFP) is calling for applications for the first phase (pre-planning) of the Community Grants Program. The pre-planning process should facilitate discussions among diverse stakeholders to identify no more than two priority health issues for their community, and how they will measure success. Applicants must submit a brief letter of interest by April 12, 2013, that describes the lead applicant and their approach to the community-driven process.
