Primary Purpose:
The Community Health Case Worker assists eligible populations through the provision of social and core services as outlined in the Cooperative Agreement to ensure that the needs of the clients in the areas of mental health, medical health and trauma healing are identified and treated. The work entails providing the clients an orientation and information for the client to access appropriate services as may be needed in a timely manner ensuring self-sufficiency.

Essential Duties:

  • Conduct orientation and complete needs assessments.
  • Coordinate aspects of pre and post arrival services including, but not limited to: healthy nutrition supplies, orientation, health screening, application for Medicaid and SNAP to ensure a safe environment and eligibility for medical services.
  • Coordinate initial Refugee medical screening as well as short and long term preventative and curative care with Resettlement and Processing (R&P) staff and the local Health Department (LHD)
  • Coordinate needed follow-up services with R&P staff and LHD staff
  • Coordinate initial refugee health screening with a primary care physician with the R&P staff, LHD staff and the department of Medical Assistance Services (DMAS)
  • Ensure the Comprehensive Resettlement Plan contains preventative and curative medical and health goals.
  • Coordinate the delivery of refugee mental health and trauma healing needs with R&P staff and local Community Services Board and other service providers.
  • Assist R&P staff with emergency refugee medical needs
  • Coordinate R&P staff, Community Service Board staff, local LHD staff and refugee training in mental health and trauma healing.
  • Coordinate staff non-professional assessment of refugee health needs.
  • Attend and participate in staff, CWS, ONS and Volag meetings and trainings.
  • Resource the local mental and health care system including health content of the Life Skills class.
  • Serve as the liaison for local health care and other agencies to provide core services provided by the R&P office as described by the Cooperative Agreement.
  • Maintain client case files and reporting for health related activities and core services.
  • Maintain confidentiality standards (HIPPA).
  • Notify supervisor of any issues with cases that require extra/unusual assistance.
  • Assist with providing transport to all required appointments in 30 day core services time frame.
  • Act as interpreter or translator for clients as appropriate and needed especially during the first 30 days.
  • Act as Client Advocate in the council and support of the clients.
  • Facilitate access and provide linkage to community health resources.
  • Create and maintain a current list of all community health resources available.
  • Maintain regular contact with clients through office and home visits.
  • Keep current, up-to-date case files on all clients.
  • Document all contacts and services in client case files and database. Complete all required reports.
  • Enter client data into Pegasus and VNIS and prepare all required reports.
  • Assist in crisis intervention as necessary.
  • Create comprehensive service plan with client and work together with client to help meet self-sufficiency goals.
  • Provide cultural and program orientation and information to clients, including information regarding rights and responsibilities, as well as federal, state and local services available to clients.
  • Collaborate with colleagues in planning and scheduling client appointments.
  • Offer interpretation and translation services to clients, as well as transportation to appointments as necessary. (Compensation is paid by CWS for mileage when using personal vehicle.)
  • Attend all staff meetings, respond to voicemails and emails in a timely manner, participate in answering the office telephone and taking messages for other staff, submit timesheets and expense claims in a timely manner, document work time on electronic time sheet, assist with furnishing refugee apartments, and participate in cleaning the office.
  • Perform other related duties as requested.
  • Work evening and/or weekend hours on occasion as required.

 

Qualifications:
Languages: Fluency in English (written and spoken) and refugee language – Arabic (spoken) preferred.
Education: High School graduate or equivalent. Degree in Human Services preferred.

Experience: Background in case management or social work desired. Minimum of 1 year experience in refugee or immigrant related services, familiarity with U.S. refugee and/or immigration-related services, providing familiarity with U.S refugee and/or immigration issues and terminology preferred. Experience working with churches or church-based organizations is preferred.
